Keto made easy

Chapter 3: Starting Keto: Month One, Water Weight and What to Expect

To start keto, treat the first month as a learning month: keep carbs low, eat enough protein, eat fat until you are full, and do not judge results until about day 60.

The short answer

Month one is about learning, not losing. You learn to read labels, shop, cook with new oils and swap in new ingredients. Your body is also adjusting to running on fat instead of carbs.

Most early weight loss is water. A stall and low energy in weeks two to four are common. Steady fat loss, if it comes, usually starts in the second month.

KetoDick on this: he calls month one a learning and adaptation period, not a weight-loss period, and says the first 30 days are for learning labels and recipes and that you should give the diet a full 60 days before judging it (video 7Aixw4uRksI; video uDw65ERtsA0).

The four month-one rules

Rule What it means in practice Where he says it
Count carbs Keep carbs low every day. This is the one number to watch closely. video DoSUgIr2rNo
Get protein Hit a protein target first at each meal. video 7Aixw4uRksI
Eat fat to fullness If you are still hungry after protein and vegetables, add fat. video 7Aixw4uRksI
Do not restrict calories Eat until satisfied. Do not set a weight-loss calorie goal yet. video DoSUgIr2rNo

Count carbs

Carbs are the lever that makes the diet keto. His carb ceilings differ by video. In 2017 he described under 20 g a day as the level for ketosis (video DoSUgIr2rNo). In his 2018 basics video his beginner rule is under 50 g of total carbs, mostly from green vegetables (video UygKGoP_BtA). The carbs chapter covers how to choose a number.

Get protein

His 2018 targets are about 80 g a day for women and 100 g for men, plus 20 g if you are very tall and 20 g more for hard training (video UygKGoP_BtA). For comparison, the US adult RDA is 0.80 g per kg of body weight per day (IOM 2005).

Protein may also help you eat less without trying. In one 12-week study of 19 adults, raising protein from 15% to 30% of energy lowered how much people ate on their own by 441 kcal a day (Weigle et al. 2005). See how much protein.

Eat fat to fullness, and do not cut calories yet

In month one, fat is the filler. Eat it to stay full while your appetite adjusts. Later, in the weight-loss phase, fat is the part that comes down (video 7Aixw4uRksI; video UygKGoP_BtA). See how much fat.

KetoDick on this: his most common beginner mistake is setting a tracking app to "lose 1.5 lb a week," which builds in a big calorie cut from day one. He says to set it to maintain weight instead (video DoSUgIr2rNo). He also says, in the auto-caption's words, "if you willpower your way through keto you're probably doing it wrong."

Two ways in: taper or start all at once

Approach How it works Trade-off
Taper Cut starchy sides first, then sugary drinks and sauces, then go to keto. Or step down 150 → 100 → 50 → 20 g of carbs. Slower, but gentler on energy and easier for families.
All at once Go to keto-level carbs on day one. Faster, but more likely to feel rough in the first two weeks.

He recommends tapering in one video and says he did not taper himself (video qfy0IBkPF4A; video cTGYIFOTPl8). Full steps: Easing into keto: the tapering method.

What the scale does in month one

Period What is usually happening His advice
Week 1 Water loss as stored carbs (glycogen) are used up Do not count it as fat
Weeks 2–4 Stall, low energy, gut changes Normal; keep going
Month 2 on Slower, steadier fat loss Weigh weekly or every two weeks

He says glycogen is stored with about twice its weight in water, so the first-week drop is mostly water and comes back quickly if you eat high-carb again (video uDw65ERtsA0). He suggests not weighing at all for the first 30 days (video uDw65ERtsA0).

Scale weight and body fat are different measures. In a metabolic-ward study of 19 adults that measured body fat directly, carbohydrate restriction led to 53 ± 6 g a day of body-fat loss, against 89 ± 6 g a day with fat restriction (Hall et al. 2015).

How you may feel

Low energy, poor sleep, headaches and cramps are common in the first weeks. An analysis of online posts found that reported "keto flu" symptoms clustered in the first weeks and were mostly described as short-lived (Bostock et al. 2020).

KetoDick on this: he splits these symptoms into two causes. Tiredness and headaches come from the energy gap before your body is fat adapted. Cramps come from low electrolytes (video DoSUgIr2rNo). For sodium he suggests salted broth or bouillon once or twice a day (video UygKGoP_BtA). See electrolytes, keto flu and fat adaptation.

What to track while you learn

Track carbs every day in month one. Track protein too, at least until you can estimate it by eye. Calories can wait.

Track Month one Later
Carbs Yes, daily When stalled or after a change
Protein Yes, until you know your portions Spot-check
Calories Optional Useful when diagnosing a stall
Weight Not yet Weekly or every two weeks

KetoDick on this: he recommends tracking food while learning because it helps you find problems, and says busy people can track carbs only (video LbPnm9T0oxc). Full guide: Tracking on keto.

Where his view and the research differ

Calories. He says studies found counting calories mattered for low-fat groups but not low-carb groups (video LbPnm9T0oxc). The evidence pages behind this guide do not include those studies. They do include a ward study in which cutting carbs did not speed body-fat loss over cutting fat (Hall et al. 2015). He also says a calorie deficit is still required (video LbPnm9T0oxc).

Pace. He expects fat loss of about 0.5–2 lb a week from month two (video UygKGoP_BtA). Average trial results are smaller over a year. In DIETFITS, 609 adults lost 6.0 kg on healthy low-carb and 5.3 kg on healthy low-fat over 12 months (Gardner et al. 2018). In another trial, low-carb lost more at 3 and 6 months, but the gap was not significant at 12 months (Foster et al. 2003). Averages hide wide individual ranges.
